A v\u00edte, \u017ee i na chronickou bolest zad se m\u016f\u017eete pojistit?<\/b><\/p>\n<p><i><span style=\"font-weight: 400;\">\u201eBav\u00ed m\u011b vym\u00fd\u0161let individu\u00e1ln\u00ed postupy, jak doj\u00edt ke spole\u010dn\u00e9mu c\u00edli. Prvn\u00ed b\u00fdv\u00e1 zbavit se bolesti, druh\u00fd dostat se z ordinace do t\u011blocvi\u010dny, kde m\u016f\u017eeme aktivn\u011b pracovat na tom, aby se do budoucna \u017e\u00e1dn\u00e9 bolesti neobjevovaly,\u201c<\/span><\/i><span style=\"font-weight: 400;\"> \u0159\u00edk\u00e1 fyzioterapeutka Vendula Nov\u00e1kov\u00e1.\u00a0 \u010clenka t\u00fdmu Martiny Dubovsk\u00e9, \u010desk\u00e9 reprezentantky ve slalomu, p\u016fsob\u00ed tak\u00e9 na pra\u017esk\u00e9 klinice VO2MAX, kam nastoupila hned po st\u00e1tnic\u00edch na 1. l\u00e9ka\u0159sk\u00e9 fakult\u011b Univerzity Karlovy. Jak sama \u0159\u00edk\u00e1, zbavovat klienty bolesti je pro ni velmi napl\u0148uj\u00edc\u00ed pocit.<\/span><\/p>\n<h2><strong>\u201eM\u00e1m pocit, \u017ee jsme se s\u00a0bolest\u00ed zad n\u011bjak\u00fdm zp\u016fsobem nau\u010dili \u017e\u00edt a tolerujeme ji. \u0158e\u0161\u00edme to, a\u017e kdy\u017e je \u00fapln\u011b nejh\u016f\u0159.\u201c<\/strong><\/h2>\n<p><b>S jakou konkr\u00e9tn\u00ed bolest\u00ed zad k v\u00e1m klienti nej\u010dast\u011bji chod\u00ed?\u00a0<\/b><\/p>\n<p><span style=\"font-weight: 400;\">Vlastn\u011b je to velmi r\u016fzn\u00e9. \u010casto je to tup\u00e1 bolest v oblasti bedern\u00ed p\u00e1te\u0159e, kter\u00e1 se ve v\u011bt\u0161in\u011b p\u0159\u00edpad\u016f zhor\u0161uje s\u00a0n\u011bjak\u00fdm pohybem, typicky s p\u0159edklonem. Jin\u00fd typ mohou lid\u00e9 popisovat jako ostrou bolest vyst\u0159eluj\u00edc\u00ed do h\u00fd\u017ed\u011b, stehna nebo i n\u00ed\u017e. Kdy\u017e p\u016fjdeme k\u00a0hrudn\u00ed p\u00e1te\u0159i, tam si klienti nej\u010dast\u011bji st\u011b\u017euj\u00ed na p\u00edch\u00e1n\u00ed u lopatky nebo tenzn\u00ed bolesti. A \u010dast\u00e1 je tak\u00e9 bolest kr\u010dn\u00ed p\u00e1te\u0159e, kde \u0159e\u0161\u00edme takzvan\u00e9 \u201ezablokovan\u00e9 krky\u201c. Ostr\u00e1 bolest se v\u011bt\u0161inou prom\u00edt\u00e1 a\u017e do hlavy a zhor\u0161uje se p\u0159i pohybu hlavy k\u00a0jedn\u00e9 stran\u011b p\u0159i \u00faklonu nebo rotaci. V\u0161echny typy bolesti mohou b\u00fdt bu\u010f akutn\u00ed, nebo chronick\u00e9 a mohou m\u00edt r\u016fzn\u00e9 p\u0159\u00ed\u010diny.<\/span><\/p>\n<hr \/>\n<p><i><span style=\"font-weight: 400;\">V\u00edte, \u017ee \u017eivotn\u00ed poji\u0161t\u011bn\u00ed Mutumutu v\u00e1s na v\u0161echny typy probl\u00e9m\u016f se z\u00e1dy pojist\u00ed? V\u00a0oblasti k\u0159\u00ed\u017ee b\u00fdv\u00e1 velk\u00e9 svalov\u00e9 nap\u011bt\u00ed z\u00a0r\u016fzn\u00fdch d\u016fvod\u016f. Ur\u010dit\u011b t\u00e9to oblasti ned\u011bl\u00e1 dob\u0159e jak\u00e1koli dlouhodob\u00e1 statick\u00e1 poloha. Tam mus\u00ed povrchov\u00e9 svaly makat, proto\u017ee jim nic jin\u00e9ho nezb\u00fdv\u00e1 a obratle jsou pod velk\u00fdm tlakem. Tak\u00e9 v\u00a0ordinaci s\u00a0klienty nar\u00e1\u017e\u00edme na sn\u00ed\u017eenou mobilitu ky\u010deln\u00edch kloub\u016f. Jestli\u017ee m\u00e1m zkr\u00e1cen\u00e9 ohyba\u010de ky\u010dle a chci chodit nebo b\u011bhat, ten pohyb mus\u00ed nahradit jin\u00fd segment. T\u00edm pr\u00e1v\u011b b\u00fdv\u00e1 bedern\u00ed oblast, kter\u00e1 je proto p\u0159et\u011b\u017eovan\u00e1 a n\u00e1sledn\u011b m\u016f\u017ee bolet. Mo\u017enost\u00ed je ale v\u00edc a v\u017edy je t\u0159eba naj\u00edt pravou p\u0159\u00ed\u010dinu obt\u00ed\u017e\u00ed.<\/span><\/p>\n<p><b>Opakovanou bolest\u00ed zad trp\u00ed a\u017e 80 % dosp\u011bl\u00fdch \u010cech\u016f. Co jsou podle v\u00e1s hlavn\u00ed d\u016fvody, pro\u010d tomu tak je?<\/b><\/p>\n<p><span style=\"font-weight: 400;\">Sice to nechceme moc sly\u0161et, ale je to nedostatkem r\u016fznorod\u00e9ho pohybu. Drtiv\u00e1 v\u011bt\u0161ina m\u00fdch klient\u016f m\u00e1 sedav\u00e9 zam\u011bstn\u00e1n\u00ed. To znamen\u00e1 minim\u00e1ln\u011b \u0161est, ve v\u011bt\u0161in\u011b p\u0159\u00edpad\u016f i v\u00edce hodin v kuse na jedn\u00e9 \u017eidli. V\u00a0tom lep\u0161\u00edm p\u0159\u00edpad\u011b se pak po pr\u00e1ci zvednou a jdou si na hodinu zab\u011bhat, zaplavat, zahr\u00e1t si tenis. Ur\u010dit\u011b je to lep\u0161\u00ed varianta ne\u017e sednout do auta a jet si dom\u016f lehnout na gau\u010d, ale ani to nem\u016f\u017ee dlouhodob\u011b vykompenzovat hodiny prosezen\u00e9 na \u017eidli. R\u00e1da v\u017edy sly\u0161\u00edm, kdy\u017e si klienti do kancel\u00e1\u0159\u00ed po\u0159izuj\u00ed balan\u010dn\u00ed \u010do\u010dky, velk\u00e9 gymnastick\u00e9 m\u00ed\u010de nebo v\u00fd\u0161kov\u011b nastaviteln\u00e9 stoly. Na gymnastick\u00e9m m\u00ed\u010di se d\u00e1 skv\u011ble prot\u00e1hnout, balancovat, posilovat. Stejn\u011b tak klient\u016fm doporu\u010duji roller <\/span><i><span style=\"font-weight: 400;\">(p\u011bnov\u00fd v\u00e1lec, kter\u00fd se pou\u017e\u00edv\u00e1 jako cvi\u010debn\u00ed a mas\u00e1\u017en\u00ed pom\u016fcka, pozn. red.)<\/span><\/i><span style=\"font-weight: 400;\">. <\/span><span style=\"font-weight: 400;\">P\u0159i rolov\u00e1n\u00ed doch\u00e1z\u00ed k hloubkov\u00e9 mas\u00e1\u017ei t\u011bla. <\/span><span style=\"font-weight: 400;\">Je to \u00fa\u017easn\u00fd pomocn\u00edk p\u0159i uvol\u0148ov\u00e1n\u00ed oblasti hrudn\u00ed p\u00e1te\u0159e a z\u00e1rove\u0148 si p\u00e1r minutami cvi\u010den\u00ed dok\u00e1\u017eete rozproudit krev v\u00a0t\u011ble, ani\u017e byste se zapotili.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">D\u016fle\u017eit\u00e9 je to st\u0159\u00eddat. Bolest n\u00e1m \u0159\u00edk\u00e1, \u017ee je n\u011bco v\u00a0dan\u00e9 oblasti v\u00a0nepo\u0159\u00e1dku \u2013 je tam n\u011bjak\u00fd nep\u0159im\u011b\u0159en\u00fd tlak, n\u011bco tam vad\u00ed, n\u011bco se neh\u00fdbe tak, jak m\u00e1, nebo se naopak n\u011bco h\u00fdbe v\u00edce, ne\u017e by m\u011blo. Okoln\u00ed tk\u00e1n\u011b na to v\u011bt\u0161inou reaguj\u00ed tak, \u017ee se \u201est\u00e1hnou\u201c a to m\u00edsto t\u00edm zafixuj\u00ed, jako bychom si vzali ort\u00e9zu. Kdy\u017e na tento podn\u011bt dlouhodob\u011b nereagujeme, za\u010dneme si p\u0159etv\u00e1\u0159et pohybov\u00e9 stereotypy tak, abychom tu bolavou oblast nedr\u00e1\u017edili. A tak to cel\u00e9 vznik\u00e1. S\u00a0t\u00edmhle stereotypem \u017eijeme, dokud neud\u011bl\u00e1me n\u011bjak\u00fd \u201e\u0161patn\u00fd\u201c pohyb, kter\u00fd zp\u016fsob\u00ed nap\u0159\u00edklad akutn\u00ed \u00fast\u0159el, akutn\u00ed blok\u00e1du, velkou bolest. Kdy\u017e se tohle stane, v\u011bt\u0161inou to nen\u00ed t\u00edm \u201e\u0161patn\u00fdm\u201c pohybem, ale t\u00edm, \u017ee si na probl\u00e9m zakl\u00e1d\u00e1me u\u017e dlouho a je jen ot\u00e1zka \u010dasu, kdy se to stane.<\/span><\/p>\n<hr \/>\n<p><i><span style=\"font-weight: 400;\">Prevence je z\u00e1klad. Ale kdy\u017e se i tak n\u011bco semele, je dobr\u00e9 v\u011bd\u011bt, \u017ee vy nebo va\u0161i bl\u00edzc\u00ed z\u016fstanete zaji\u0161t\u011bn\u00ed. N\u011bkdy ale i \u201epouh\u00fd\u201c akutn\u00ed \u00fast\u0159el dok\u00e1\u017ee potr\u00e1pit na dlouhou dobu, kdy\u017e se s\u00a0n\u00edm nic ned\u011bl\u00e1. Proto \u0159\u00edk\u00e1me klient\u016fm: pokud c\u00edt\u00edte jak\u00fdkoli n\u00e1znak bolesti, zablokov\u00e1n\u00ed, tuhost, p\u0159ij\u010fte. \u010c\u00edm d\u0159\u00edv p\u0159ijdete, t\u00edm l\u00e9pe se n\u00e1m bude pracovat a t\u00edm d\u0159\u00edv budete zp\u00e1tky v\u00a0provozu.<\/span><\/p>\n<p><b>D\u00e1 se chronick\u00e1 bolest vyl\u00e9\u010dit tak, \u017ee na\u0161e z\u00e1da nakonec budou zcela zdrav\u00e1?<\/b><span style=\"font-weight: 400;\">\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">M\u00fdm c\u00edlem je dostat klienty do takov\u00e9 f\u00e1ze, kdy je z\u00e1da opravdu bolet nebudou. A kdy\u017e se jednou za \u010das v\u00a0n\u011bjak\u00e9 men\u0161\u00ed m\u00ed\u0159e ozvou, budou si s\u00a0t\u00edm um\u011bt poradit. Zacvi\u010d\u00ed si p\u00e1r cvik\u016f, uprav\u00ed pracovn\u00ed pozici nebo m\u011b jednor\u00e1zov\u011b nav\u0161t\u00edv\u00ed a d\u00e1me t\u011blo zase na n\u011bjak\u00fd \u010das dohromady. To je ide\u00e1ln\u00ed pr\u016fb\u011bh. Mus\u00edme na tom ale pracovat oba, j\u00e1 i klient.<\/span><\/p>\n<p><b>Dok\u00e1zala byste \u0159\u00edct, kter\u00e9 sporty jsou pro na\u0161e zdrav\u00e1 z\u00e1da nejvhodn\u011bj\u0161\u00ed? Je n\u011bjak\u00fd sport, kter\u00fd vylo\u017een\u011b pom\u00e1h\u00e1 na bolest zad?<\/b><\/p>\n<p><span style=\"font-weight: 400;\">Ka\u017ed\u00fd sport se d\u00e1 d\u011blat kvalitn\u011b i nekvalitn\u011b z\u00a0hlediska fungov\u00e1n\u00ed pohybov\u00e9ho apar\u00e1tu. Sp\u00ed\u0161 ne\u017e poukazovat na konkr\u00e9tn\u00ed sport chci zd\u016fraznit d\u016fle\u017eitost rozcvi\u010dky. T\u011blo je pot\u0159eba v\u017edycky na pohybovou aktivitu p\u0159ipravit \u2013 proh\u0159\u00e1t, rozh\u00fdbat. Na konci sportu zase za\u0159adit prota\u017een\u00ed. Kdybych m\u011bla vybrat konkr\u00e9tn\u00ed sporty, kter\u00e9 by mohly pomoci v\u00a0prevenci bolesti zad, vybrala bych lezen\u00ed, j\u00f3gu, plav\u00e1n\u00ed, b\u011b\u017eky, b\u011bh, ch\u016fzi. Nelze to ale \u0159\u00edct obecn\u011b. Ka\u017ed\u00fd a\u0165 si vybere, co ho bav\u00ed a co mu d\u011bl\u00e1 dob\u0159e. U n\u00e1s na klinice d\u011bl\u00e1me fyziotr\u00e9ninky, kde pracujeme na kvalitn\u00edch pohybov\u00fdch stereotypech.
